~ REGISTRATION CHECKLIST  ~

Items you need to bring with you when you come to Register

& Enroll at NPHS:

1. Birth Certificate
2. Social Security Card
3. Updated immunization record on BLUE card Form 680 from the Department of Health.
4. Physical (Comprehensive) - Given within one year of entry.
5. Proof of Residency. Can be utility bill, lease agreement, or front page of settlement.
6. Custody or divorce papers (if applicable.)
7. Name, Phone number, and Fax number of previous school.
8. Withdrawal form from previous school.
9. Transcript, last report card, test scores.
10. IEP needed to schedule if special needs program.
